Medicare determines your IRMAA surcharge using your MAGI (Modified Adjusted Gross Income) from 2 years prior. So your 2024 Medicare premium is based on your 2022 income. This is why claiming SS timing matters — it affects when SS income appears in your MAGI.

Up to 85% of your SS benefit may be included in your MAGI depending on your total income. When you start collecting SS, it can push your income into a higher IRMAA tier — increasing your Medicare premiums by $60–$420/month per person.
Good News
You Can Appeal an IRMAA Determination
If your income has dropped significantly since the lookback year (e.g. you've retired), you can request a Life-Changing Event (LCE) appeal using Form SSA-44. SSA can use more recent income data, which may lower your IRMAA tier immediately.
